The Impact of Cone Beam CT (CBCT) on Therapeutic Decision-Making in Endodontics
Abstract
Cone beam computed tomography (CBCT) provides three-dimensional visualization that conventional periapical radiographs cannot offer, making it an increasingly valuable tool in endodontic diagnosis; nevertheless, its influence on clinical decisions in routine practice is not yet fully understood. The present study aimed to assess the extent to which CBCT, used in accordance with current European Commission guidelines in a normal clinical setting, influences therapeutic decisions in a population referred for endodontic problems.
Specifically, it sought to compare the preliminary treatment plan established after clinical and conventional radiographic examination with the definitive plan following CBCT assessment, to quantify the proportion of cases in which CBCT findings led to a modification of the initial decision, and to identify which clinical indications most frequently resulted in a change of treatment plan.
Data were collected from consecutively examined patients between October 2022 and June 2026. Sixty patients were referred for CBCT examination based on criteria consistent with current European guidelines, and all examinations were performed using comparable, standardized equipment across the participating clinics. Following a thorough clinical examination but prior to CBCT, the examiner formulated and recorded a preliminary treatment plan; after reviewing the CBCT images, a definitive plan was established.
Any discrepancy between the preliminary and definitive plans was documented and analyzed to determine the diagnostic and therapeutic impact of CBCT on endodontic management.

Biography
Philipp-Edmund-Viktor Stremkowski (“Phil”), born in Munich in 1997, is a dentist and qualified endodontist, having completed his endodontics residency at “Carol Davila” University of Medicine and Pharmacy in Bucharest. A graduate of the “Vasile Goldiș” University Faculty of Dentistry in Arad, he practices as an endodontist at Unique Smile Clinics (Sibiu) and the Prof.
Dr. Constantinescu Clinic (Bucharest). His expertise spans microscope-assisted endodontic treatment and retreatment, implant prosthodontics, CAD/CAM restorations, and periodontal surgery.
