Current Concepts in the Treatment of Acute Periodontal Problems
Abstract
There are a few acute clinical periodontal conditions that may need prompt intervention in the dental clinic. Necrotizing periodontal diseases, endodontic-periodontal combined lesions, and periodontal abscesses are among these conditions. Patients with these problems usually seek treatment due to the major complaint of pain. The main emergency intervention for periodontal abscess is drainage followed by elimination of the cause.
Endodontic-periodontal combined lesions need to be treated both endodontically and periodontally and endodontic treatment has the priority in the majority of cases. Management of necrotizing periodontal diseases mainly relies on mechanical debridement and oral antiseptics such as chlorhexidine and hydrogen peroxide with systemic antimicrobials reserved for cases with systemic symptoms.
Management of the acute condition should be followed by management of the pre-existing periodontal disease, predisposing factors, and the potential sequela. Research on adjunctive treatment modalities such as photodynamic therapy are ongoing.

- Establish the differential diagnosis of these conditions based on their characteristic clinical and radiographic findings.
